Waivers & Consent

For the safety and enjoyment of all children, Party with Sammie requires that parents or guardians complete and sign the following forms:

Waiver Form – Glitter Tattoos
Consent Form – Glitter Tattoos
Consent Form – Photos & Video

All three forms are on one downloaded document which you will find below.

The form must be signed by a parent or legal guardian for each participating child.

Completed forms are to be returned no later than 3 days before the scheduled party.

This ensures:

Adequate time to review any medical/allergy concerns.
Proper consent for activities and media use.
A smooth, safe, and fun experience for every participant.

Please note: Children who do not have signed forms on file will not be permitted to participate in glitter tattoos or be included in photos/videos.
